Job Summary:

The Rapid Re-Housing FIVCO Service Coordinator is a full-time professional position responsible for assisting homeless program participants in securing housing and stable income. This role involves ongoing assessment, support, and coordination of community resources to meet individual participant needs, aligned with Welcome House Core Values: Compassion, Advocacy, Integrity, Quality, Leadership, and Community.

Principal Duties & Responsibilities:

  1. Service Coordination:
    • Maintain a caseload of up to 25 program participants.
    • Attend Local Prioritization Committee meetings for program referrals.
    • Assess housing barriers, needs, and preferences to develop personalized action plans.
    • Assist participants with housing search and provide coaching on landlord interactions.
    • Resolve tenancy issues collaboratively with landlords to maintain housing stability.
    • Conduct inspections of potential housing units for compliance.
    • Conduct regular home visits (at a minimum bi-monthly) and budget reviews with participants.
    • Coordinate access to government and nonprofit services.
    • Advocate for participants and empower self-advocacy.
    • Maintain accurate documentation and enter data into VESTA database.
  2. Housing Search:
    • Assist with housing search, contacting landlords, scheduling showings, and providing leads.
    • Educate and recruit landlords to participate in Welcome House programs.
    • Collaborate closely with Regional Housing Specialist during lease-up processes.
  3. Internal/External Collaboration:
    • Represent the agency at external meetings.
    • Maintain positive community relations and act as a liaison with related services.
  4. Administration:
    • Complete required documentation for HMIS and maintain files per agency guidelines.
    • Review and audit program files as directed.
    • Stay informed on best practices and maintain knowledge base.
  5. Other Duties:
    • Perform additional tasks as assigned by Regional Supervisor or Director of Regional Services.

Qualifications:

  • Education: Minimum Associate Degree to Bachelor’s Degree preferred.
  • Experience: 1-3 years in social services.
