JOB SUMMARY

The Peer Support Service Coordinator is a pivotal full-time role dedicated to delivering direct services, focused on case management and advocating for program participants experiencing or at risk of homelessness. Responsibilities include collaboratively developing tailored case plans and goals to secure stable income and housing. Continuous assessment and supportive guidance are provided to individuals with mental health and substance use challenges, facilitating their access to essential community resources. PRINCIPAL DUTIES & RESPONSIBILITIES

1. Service Coordination

·       Manage a caseload of up to 40 high-need program participants experiencing mental illness and/or substance use disorder, providing intensive, individualized support and case management.

·       Regularly meet with program participants in their units or in the office to discuss and review issues that impact income and housing, review service plans for stability and self-determination.

·       Conducts mandatory bi-monthly home visits but could be more based on acuity to discuss and review issues that impact income and housing with each program participant.

·       Coordinates program participants’ access to government services and other non-profit services.

·       Advocates for program participants when necessary, and empowers them to advocate for themselves, when appropriate.

·       Complete initial applications, checklists, Vesta intake, and associated paperwork, ensuring all documents are uploaded into Vesta within 3 business days from intake.

·       Maintains accurate and up-to-date documentation of program participant service plans, case notes, and change of status documents and other reporting and tracking forms.

·       Ensures ongoing data entry and document upload into Vesta within 3 business days.

2.  Outreach

·       Provide outreach services to the individuals experiencing homelessness in the community, on the streets, in encampments and in the Buffalo Trace counties as deemed necessary by the Director of Regional Services East and/or Supervisor of Sutton’s Landing.

·       Responsible for providing documentation of homelessness.

·       Provides transportation for program participant’s/program participant families, as needed.

·       Collaborate with the Welcome House Street Outreach team to assist in connecting program participants to services.

3. Internal/External Collaboration

·       Attends All Agency or any agency related mandatory meetings or trainings, team huddles, program team meetings, department meetings and supervisions.

·       Collaborates with the Welcome House Sutton’s Landing Support Staff to assist program participants in accessing resources.

·       Works as a part of a Sutton’s Landing team with Welcome House Outreach to provide a continuum of quality services to program participants.

·       Provides respectful and collaborative support to service team members and shares responsibility with all team members for program participant outcomes.

·       Represents the agency, Sutton’s Landings Team and the Director/Supervisor of Regional Services East at outside agency meetings on an as needed basis.

·       Maintains positive public relations and serves as a liaison with other related community services.

4. Administration

·       Maintains up to date program participant list.

·       Complete annual participant assessments in Vesta

  • Enter all ongoing participant data and documentation into the Vesta database within three business days.

·       Upload receipts into the BILL system for payments made by credit card.

·       Keeps accurate data on all program participants and assists the Director of Regional Services East with data gathering and reporting, as needed.

·       Provides information to the Director of Regional Services East for quarterly and annual reporting purposes.

·       Provides analysis of data and outcomes to the Director of Regional Services East, when requested.

5. Other duties as specified by the Director/Supervisor of Sutton’s Landing

 

 

QUALIFICATIONS:

           Valid Driver’s License & Automobile Insurance

Education:

  • Bachelor’s Degree in a related field of Social Work, Human Services, Sociology, or Psychology preferred.
  • Peer Support Certification

Experience:

  • A minimum of 1 – 3 years’ experience in the social service field
  • Prior experience working with individuals and families experiencing homelessness preferred. Case management experience is a plus. Experience in homelessness, home visits, substance abuse, or mental health services is desired.

 

Skills/Specialized Knowledge/Abilities

  • Ability to maintain clear personal and professional boundaries with participants.
  • Ability to work effectively in changing and demanding environments.
  • Prior experience using the Homeless Management Information System (HMIS) is a plus.
  • Proficient in problem solving techniques and have a high capacity to think critically
  • Proficient ability to communicate both verbally and in writing
  • Proficient ability in conflict management
  • Advanced to work independently as well as part of a team
  • Advanced ability to develop community resources and networking ability
  • Increased knowledge of and involvement in community resources/partnerships
  • Proficient knowledge of Mental Health/Substance Abuse resources
  • Ability to collaborate both internally and externally
  • Must possess a valid driver’s license and car insurance

WORKING CONDITIONS:

Working Hours/Environment

  • Office hours: Monday through Friday, 8:30 a.m. – 5 p.m.
  • Work performed indoors, either in the participant’s home or at the agency office, off-site meeting locations, and some duties performed outdoors.

 

Tools and Equipment Used

  • Personal computer, copier, fax/scanner, phone, and other typical office equipment.

 

Travel

  • Moderate-approximately 30% of the time.

 

Physical and Mental Demands

  • Work often done at off-site locations, including occasionally outdoors
  • Required to sit at a desk/workstation
  • Ability to work at a computer terminal for extended periods of time
  • Digital dexterity and hand/eye coordination in operation of office equipment
  • Ability to speak to and hear employees/participants via phone or in person
  • Body motor skills sufficient to enable incumbent to move between 3 floors and conduct home visits in various settings
