The Mental Health Service Coordinator is a full-time professional, direct service position responsible for providing case management and program participant advocacy by working with program participants who are experiencing homelessness or at risk of experiencing homelessness to develop appropriate case plans and goals for obtaining stable income and housing. This position also provides on-going assessment and support to program participants with mental illness and helps program participants utilize community resources to meet their individual needs. This position performs duties in accordance with Welcome House Core Values, including: Compassion, Advocacy, Integrity, Quality, Leadership and Community.
PRINCIPAL DUTIES & RESPONSIBILITIES
1. Service Coordination
• Maintain caseload of 15-20 intensive program participants with mental illness.
• Regularly meets (as needed) with program participants off site or in the office to discuss and review issues that impact program participant income and housing, review program participant service plans for stability and self-determination.
• Conducts mandatory bi-monthly home visits but could be more based on acuity to discuss and review issues that impact income and housing with each program participant and in the month prior to HUD recertification to ensure compliance with HUD requirements.
• Coordinates program participants’ access to government services and other non-profit services.
• Advocates for program participants when necessary, and empowers program participants to advocate for themselves, when appropriate.
• Maintains accurate and up-to-date documentation of program participant service plans, case notes, and change of status documents and other reporting and tracking forms.
• Uploads initial documents filled out the program participants at project entry within 5 business days in the Vesta database.
• Ensures ongoing data entry and document upload into Vesta within 3 business days.
2. Outreach
• Provide outreach services to the individuals experiencing homelessness in the community, on the streets, in encampments and in the Northern Kentucky rural counties as deemed necessary by the Director of Homeless Solutions and/or Supervisor of Homeless Solutions.
• Responsible for providing documentation of homelessness in collaboration with the Service Coordination team.
• Provides transportation for program participant’s/program participant families, as needed.
• Collaborate with the Welcome House Street Outreach team to assist in connecting program participants to services.
3. Internal/External Collaboration
• Collaborates with the Welcome House Street Outreach to assist program participants in accessing resources.
• Works as a part of a Service Coordination team with Welcome House Outreach, Soar, shelter services and the Department of Housing and Supportive Services to provide a continuum of quality services to program participants.
• Provides respectful and collaborative support to service team members and shares responsibility with all team members for program participant outcomes.
• Represents the agency, Service Coordination team, Supervisor and/or Director of Homeless Solutions at outside agency meetings on an as needed basis.
• Maintains positive public relations and serves as a liaison with other related community services.
4. Administration
• Maintains up to date program participant list.
• Keeps accurate data on all program participants and assists the Director of Homeless Solutions with data gathering and reporting, as needed.
• Provides information to the Director of Homeless Solutions for quarterly and annual reporting purposes.
• Provides analysis of data and outcomes to the Director of Homeless Solutions, when requested.
5. Other duties as specified by the Supervisor/Director of Homeless Solutions.
QUALIFICATIONS:
Education:
• Bachelor’s Degree required, Master’s Degree and Licensee desired or equivalent
Experience:
• A minimum of 3-5 years’ experience in the social service field
Skill Set:
• Proficient in problem solving techniques and have a high capacity to think critically
• Proficient ability to communicate both verbally and in writing
• Proficient ability in conflict management
• Advanced to work independently as well as part of a team
• Advanced ability to develop community resources and networking ability
• Increased knowledge of and involvement in community resources/partnerships
• Proficient knowledge of Mental Health/Substance Abuse resources

WORKING CONDITIONS:
Working Hours/Environment
• Office hours: Monday through Friday, 8 a.m. – 5 p.m.
• Work typically performed in an indoor office building.
Tools and Equipment Used
• Personal computer, copier, fax/scanner, phone, and other typical office equipment.
Travel
• Moderate-approximately 30% of the time.
Physical and Mental Demands
• Frequently required to sit at a desk/workstation for long period of time
• Ability to work at a computer terminal for extended periods of time
• Digital dexterity and hand/eye coordination in operation of office equipment
• Ability to speak to and hear employee’s/program participants via phone or in person
• Body motor skills sufficient to enable incumbent to move between 3 floors
• Additional Mental Requirements: compare, decide, direct, problem solve, analyze, instruct, interpret
• Minimum lifting up to and above 50 pounds
