The Family Case Management program partners with two other Northern Kentucky agencies in comprehensive, interdisciplinary, holistic services for homeless families and women. The goals are to secure sufficient income, foster self-determination and stability, and assist in obtaining and maintaining permanent housing for those who have struggled to sustain these basic needs due to mental illness, domestic violence, and/or chemical dependency issues. Many of those we serve (88%) deal with two or more life altering disabilities or obstacles that affect their ability to maintain work and housing.
Total served in case management in 2007: 669 individuals; (104 single individuals, 198 parents and 367 children). 41% of the children (17 & under) were birth to 5 years old. An additional 172 families/month are served in outreach and follow up services, along with housing and budget counseling and crisis intervention by phone.
- 100% of all adults and children entering the program are assessed for substance abuse, domestic violence, and mental health needs. All adults are assessed for income/employment, childcare, and housing needs and barriers. They are then provided and referred to the appropriate service to assist in reaching their goals.
- 75% of case management clients obtained permanent housing manageable within their existing budget.
- Of the families obtaining housing the prior year (2006), 77% maintained their housing for at least six months or longer.
- 81% of those entering the program accessed and obtained benefits for which they were eligible. This includes basic medical, financial and food stamp & nutritional benefits made available for low income individuals and families.
